The Clinical and Psychosocial Support Department (CPSD) Case Manager Supervisor, and HIV/STI Prevention Case Manager (CM), provides supervision to the CPSD case managers. This role also provides HIV/STI prevention case management, education, referrals, linkage, and coordination of care to internal and external support services and providers and supports the patient’s medical and psychosocial plan of care. This position reports to the Senior Director of Clinical Education and Initiatives.

Essential Functions:
1. Provide individual and relationship-based HIV and STI counseling, risk reduction strategies, education on pre-exposure prophylaxis (PrEP), non-occupational post exposure prophylaxis (nPEP) and other effective prevention interventions. Provides appropriate resources, referrals and facilitate opportunities for testing.
2. Assist patients in identifying the level of risk for HIV acquisition through a patient centered approach. Supports patients in sexual identity, orientation, experience, and needs.
3. Provide population specific education to those with increased vulnerability to HIV with a focus on cisgender black women, trans- and gender non-conforming (TGNC) communities, young men who have sex with men (YMSM).
4. Provide case management and care coordination to facilitate a person- centered comprehensive HIV/STI prevention plan and risk reduction strategy for people vulnerable or at an increased risk to HIV.
5. Incorporate PrEP-Ception and post-partum HIV prevention counseling in patients with pregnancy potential, interested in family planning with a serodifferent partner or partner with unknow HIV status and/or a desire to breast/chest feed. Collaborates with the CPSD Prenatal Bridges CM for family planning and referral.
6. Provide adherence counseling for patients on PrEP or nPEP, assist in scheduling initial/follow-up appointments, promote and track retention and compliance and care re-engagement.
7. Assist patients and clinicians in obtaining PrEP, nPEP or rapid HIV treatment by enrolling into patient assistance programs. Assist in completing and tracking required paperwork for Co-Pay cards, medication and tracking timelines for re-enrollment.
8. Ensure patient’s access to health insurance and refer to PPIL insurance specialist.
9. In collaboration with the medical clinician, refer and link patient with a positive HIV test to an Infectious Disease or HIV provider. Provide case management until patient has attended their initial medical visit. Obtain patient release of information, assist in transferring medical records and lab tests to referring provider and document in the electronic medical record.
10. Act as a PrEP ambassador for PPIL staff, internal and external stakeholders and the community through community engagement, outreach, developing partnerships and providing training and education on the basics of PrEP to ensure accurate knowledge.
11. Review and maintain an up-to-date and accurate knowledge of emerging scientific research regarding biomedical HIV prevention strategies, including PrEP, nPEP, and related interventions.
12. Attend meetings and trainings required by grant funders.
13. Ensure the proper collection, storage, and transmission of data used in reporting, research, and program evaluation. Accurately complete all statistical data forms for PrEP, primarily within the Electronic Health Records System.
14. Obtain prior authorization for medications and provider visit. Understand the process for submission, working with Cover My Meds and navigating denials.
15. Maintain log of patient assistance, prior authorization requests and denials. Identify trends and report unusual findings to supervisor for further investigation.
16. Develop a medical and psycho-social plan in conjunction with the CPSD team, to assist the patient in successfully meeting desired care goals.
17. Refer patient in crisis or in need of short-term counseling to a behavioral health clinician (BHC).
18. Advocate for patients to ensure their voices heard and acted upon in the scope of care provided.
19. Assist team in creating a resource and referral lists for PrEP access in cis-gender black women and LGBTQ communities to ensure access to safe spaces, valuable resources, competent providers, and friendly LGBTQ organizations.
20. Provide information on PPIL full sexual and reproductive health care services in addition to PrEP, nPEP, gender affirming hormone care and safe/safer sex practices.
21. Demonstrate an understanding of and commitment to remain informed about PPIL protocols, policies, and procedures.
22. Demonstrate an understanding of and commitment to PPIL core values of access, activism, care, confidentiality, diversity, excellence, integrity, respect, self-determination, and stewardship; practice these values in relations with internal and external customers.

Supervisor Responsibilities:
1. Provide case management program support.
2. Coach, mentor and provide guidance and supervision to the CPSD CMs.
3. Provide training and evaluation of CMs according to their specialty area, including HIV/STI prevention, PrEP/nPEP, GAHT care, and Prenatal Bridges CM.
4. Work with the supervisor to create, review and implement case management policies and procedures.
5. Monitor and assist with prioritizing and organizing patient requests among the CMs.
6. Work in collaboration with Senior Director of Clinical Education and Initiatives in the interview process of new hires and orientation.
7. Perform clinical chart audit.
8. Schedule and organize staff meetings.
9. Serve as point person for requests for internal and external resources.
10. Attend intradepartmental, managers and clinician meetings.

Other Responsibilities:
1. Provide education and serve as a consultant for HIV/STI prevention with PrEP to staff.
2. Work closely with the Centralized Follow-Up team to assist with calls, communications, and follow-up plans for patients seeking assistance.
3. Keep records in accordance with documentation standards.
4. Manage confidentiality of clients; work with law enforcement agencies when required; activate the community mental health system when required.
5. Collect program planning data as directed.
6. Attend monthly CPSD meetings and case manager meetings.
7. Consult with CPSD team regarding specific cases as appropriate. Participates in relevant PPIL staff meetings.
8. Other duties as assigned.

Supervisor: Senior Director of Clinical Education and Initiatives
Status: Full time. Non-exempt from the overtime provisions of the wage and salary regulations.

4. Principles and objectives of STI/HIV Prevention with PrEP.
5. Experience with screening and counseling on comprehensive risk reduction strategies.
6. Accessing PrEP and nPEP through patient assistance program.
7. Community organizations and resources focusing on ending the HIV epidemic.
8. Principles and objectives of psycho-social needs for transgender individuals.
9. Medical-psycho-socio-economic factors affecting transgender, and gender non-conforming individuals including minors accessing GAHT.
10. Community organizations and resources focusing on transgender or LGBT communities.
11. Rules and regulations affecting the delivery of service in health care agencies, including, but not limited to, patient confidentiality, DCFS (child abuse/neglect), domestic violence, and duty to warn.

Education and Experience: Bachelor’s degree from an accredited college. Registered Nurse preferred. 4 years’ experience working in the STI/HIV Prevention, educating on PrEP and nPEP as part of an HIV prevention strategy. 2 years supervisory experience. Experience working with vulnerable populations with a focus on cisgender black women, MSM and the LGBTQ community. Experience with prior authorizations for medical case management and PrEP or other medical patient assistance programs. Experience in gender affirming care.

An ability to communicate with clients, paid and volunteer staff, and the public in a sensitive, professional manner regarding gender concerns. Well-developed writing skills. Willingness to work a flexible schedule, including some evenings and weekends. Must have a car and be willing to travel throughout Illinois to all PPIL sites. PPIL reimburses for mileage and other relevant travel expenses. Bilingual ability in Spanish or other languages is preferred but not required.
