Position Summary
The Patient Navigator provides case management and social work services to patients across Massachusetts, helping them access the medical care and community resources they need. This position works closely with multidisciplinary teams and be a trusted advocate for patients navigating complex systems. This role is ideal for someone deeply committed to health equity, racial justice, and community empowerment. A successful candidate will be passionate about supporting patients through compassionate, culturally responsive care.

Candidates must reliably commute to Massachusetts clinic locations

Essential Responsibilities:

    • Conduct assessments and document patient progress in electronic health records.
    • Provide short-term counseling and connect patients to community-based resources.
    • Offer crisis intervention and support to high-risk individuals.
    • Collaborate with care teams and community partners to ensure holistic support.
    • Maintain up-to-date referral lists and resource guides.
    • Represent PPLM in community and public health meetings.
    • Support the Abortion Access Program and other patient-centered initiatives.
    • Serve as a Certified Application Counselor for Massachusetts health insurance programs.
    • Participate in weekly supervision and contribute to organizational goals.

Qualifications:

    • ✅ 1–3 years of experience in social services, preferably in diverse communities.
    • ✅BSW or MSW preferred; relevant experience highly valued.
    • ✅Bilingual in Spanish and English strongly preferred.
    • ✅ Flexible schedule, including availability for evenings and weekends.
    • ✅ Reliable transportation, valid driver’s license, and willingness to travel statewide.
    • ✅ Proficiency in Microsoft Office Suite and electronic health records.
    • ✅ Experience in crisis intervention, case management, counseling, or advocacy.
    • ✅ Familiarity with Massachusetts health insurance programs is a plus.
$25.50 - $25.50 an hour
This position is part to the Collective Bargaining Agreement
