Position Reports to:    Director of Nursing

Summary: Planned Parenthood seeks a Centralized Follow-Up LPN. Under the general supervision and direction of the Director of Nursing and Director of CRQM, the RN/LPN provides quality patient care in accordance with delegated responsibilities and agency policies and functions professionally to ensure that agency medical standards are met. The Centralized Follow-Up LPN will follow up on STI/Pap management plans for all health centers.

Qualification & Experience

    • Graduate of an accredited school of nursing for LPN and holds a LPN License in the State of New Jersey. 
    • 1-2 years of experience in women’s health care, obstetrics, public health or related area preferred.
    • Applicant must be customer service focused and committed to the mission of the organization
    • Must be able to utilize sound judgment in assessing patient needs.

Essential Functions/Responsibilities

    • Follow-up Nurse
    • Conducts follow-up on designated treatment/management plans initiated by clinicians which includes:
    • Running designated reports at least three times a week to identify and track follow-up activities.
    • Contacting clients by phone and mail.
    • Explaining test results and requisite treatment and follow-up
    • Documenting appropriately in the EMR system all client communication
    • Monitors the timeframe of clinicians’ initiation of management plan 
    • Communicate with clinicians, Health center managers, call center staff, and supervisors as needed.
    • Works in an efficient manner and completes tasks and projects on time.

    • Float Nurse
    • Provides nursing coverage at PPMNJ clinics as needed.
    • Obtains medical/social history and vital signs.
    • Interprets laboratory and diagnostic procedures and findings.
    • Performs laboratory tests as needed.
    • Dispenses supplies and administers medications.
    • Triages medical calls.
    • Provides education and counseling in a non-judgmental manner to patients by providing support and information on reproductive and sexual health issues, including birth control methods, pregnancy options, prenatal care, abortion, STI’s/HIV and safer sex.
    • 9.Maintains appropriate patient records.
    • 8.Ascertains charges and keeps accurate account of monies.
    • 9.Assists client in making medical and social referrals.
    • 10.Participates in quality assurance activities and meets established QA deadlines.
    • 11.Ensures compliance with PPMNJ’s “Privacy Guidelines” and HIPAA compliance program.
    • 12.Assumes additional responsibilities and performs other delegated duties 
    • 13.Attends meetings as deemed appropriate by the supervisors

$30.81 - $34.47 an hour
