Job Purpose:
We are seeking a dedicated and compassionate Home Care Nurse to join our team in Little Grand Rapids First Nation. This position is full-time (1 FTE), but consideration may be given to applicants looking for only part-time (.5 FTE).
Under the direction of the Home and Community Care Coordinator, this role involves providing essential healthcare services to clients in their homes.
Duties and Responsibilities:
- Deliver high-quality nursing care to homebound clients.
- Conduct comprehensive health assessments and develop individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor patients' conditions and respond to changes in health status.
- Provide wound care, injections, and other clinical procedures as needed.
- Educate clients and their families on healthcare management, disease prevention, and healthy lifestyle choices.
- Respond promptly to medical emergencies and provide appropriate interventions.
- Coordinate emergency patient transportation if necessary.
- Maintain up-to-date knowledge of emergency protocols and procedures.
- Maintain accurate and up-to-date client records.
- Prepare and submit reports on client care and program outcomes.
- Ensure compliance with regulatory requirements and organizational policies.

Qualifications/Requirements:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license to practice in Manitoba.
- Experience in home care or community health nursing is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Commitment to providing compassionate and client-centered care.
- Valid driver's license and access to a reliable vehicle.
- Valid Emergency First Aid/CPR certification.
- Certification in Advanced Cardiovascular Life Support (ACLS)
- Experience in emergency response or critical care settings.
- Knowledge of local health services and community resources in Little Grand Rapids First Nation.
- Experience in program coordination and staff supervision.
- Ability to speak and understand Indigenous languages relevant to the Little Grand Rapids community.

Working Conditions:
- Willing and able to travel to remote first nations communities as required, by all forms of transportation, (must have access to a reliable insured vehicle) and valid Manitoba Driver’s License.
- Additional hours beyond the standard regular hours of work may be required at times.
- Ability to lift at least 50lbs.
