As a Registered Nurse Care Manager, you will focus on the care management of patients with acute and chronic medical conditions the assessment of the care needs of vulnerable populations for various clients.
Essential Functions
- You will perform assessment and evaluation of potential care management patients per the client's contractual obligations and/or state requirements.
- You will collaborate with health care providers and ancillary service providers to assure coordination of care to ensure effectiveness of treatment plans and identify educational opportunities.
- You will provide ongoing education, resource information, and support to patients that reinforce their treatment plan and enables them to improve their health management skills.
- You will evaluate treatment adherence, member safety, and socioeconomic factors, and re-evaluate the patient’s commitment to making the necessary changes on a regular basis.
- You will document per contractual requirements related to individual member status, and/or medical necessity review, and negotiation.
Requirements
- Four-year degree in health care or related field and/or equivalent training and/or experience.
- Current unrestricted RN license required.
- Minimum 1-year experience in human services and/or 3 years of clinical practice experience.
- Case management and/or Mental Health experience preferred.
- Certified Case Manager (CCM) strongly preferred or ability to obtain CCM once becoming a Telligen employee-owner.
