Washburn Center’s case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family’s needs and coordinate care across multiple providers.  These services are provided for children age 3 through 17.

Additionally this role is a highly collaborative function, and you will experience exposure to all programs at Washburn Center and work in collaboration with the various experienced professionals at Washburn to ensure the best care for the children and families in your care.
 
The case management professional will be actively present in the community and the home life of the client.  We encourage independent working for all our case managers to allow for the best client care and services. 
 
ROLE RESPONSIBILITIES:
1.       You will be managing a caseload of 15-18 children or youth and their families with all client service expectations that accompanies this caseload.
2.       At Washburn Center for Children we are committed to provide culturally based, individualized targeted Children’s Mental Health Case Management to children/students and families in their home, schools and community based on each family’s needs. 
3.       You will coordinate mental health and other health related services.  The purpose of this involvement in the child and family’s life is to improve or maintain child/student’s emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services.
4.       You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes, case management authorizations, and treatment plans etc.
 
DAY TO DAY ACTIVITIES:
Clinical Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:
Assessment – for your clients you will complete a functional and diagnostic assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.
Service Plan Development – You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis.
Dependent on each individual case you will refer, coordinate, and monitor services

EDUCATION, LICENSURE & SKILLS

    • Must have a Master’s degree in Social Work, Counseling or Psychology
    • If candidate has a Master’s in Social Work, valid licensure as an LGSW is required
    • A minimum of one year of experience with children and families in acute mental health distress is required
    • Flexibility, resourcefulness, and demonstrated cultural responsiveness when working with diverse populations, specifically those impacted by systemic inequality
    • Knowledge of community resources and experience with home and community-based services
    • Must have vehicle in operating condition, a valid driver’s license, and car insurance
$52,000 - $58,000 a year
Salary Bracket: $52,000 - $58,000 (Salary is based on several factors including but not limited to licensure, work experience and education)

Vacation:  Paid Vacation accrues at 10 hours of vacation per month - up to 15 days / 120 hours annually.
Sick Leave:  Paid sick leave accrues at 6.68 hours of sick per month - up to 10 days / 80 hours.
Holidays:  There are 2.5 days/20 hours of floating holidays per year in addition to the eight standard holidays.
