World Relief seeks a Refugee Health Coordinator.  The focus of this position is on educating and equipping all new refugee & asylee clients with the understanding and practical skills needed to navigate healthcare settings in the United States. This position also focuses on connecting each newly arriving refugee and those granted asylum with a primary care provider through a health screening which requires a significant amount of driving. The position is based in Aurora and requires frequent client home visits throughout Kane and surrounding counties and is grant-funded.

Role & Responsibilities

    • Health Care Coordination:
    • Collaborate with Refugee Health Specialist around the coordination of Health Screenings which may include scheduling visits for each new arrival and/or asylee
    • Provide transportation for the initial and follow-up appointments, and additional appointments as needed per client. 10 to 20 hours a week will be spent on driving clients each week to necessary health screening and related medical appointments.
    • Assist clients in creating a transportation plan by leveraging community resources, rideshare, or public transportation for transportation to follow-up appointments
    • Assist clients in navigating health insurance requirements in collaboration with other WR staff for accessing medical specialist care and educate clients on how to work with health insurance to pay medical bills
    • Coordinate with outside agencies/organizations to meet client needs, advocating and mediating on behalf of clients, as needed
    • Coordinate with primary care providers, specialist medical providers, and medical insurance providers to ensure that clients receive appropriate, timely medical care and appropriate language interpretation
    • Empowering clients towards self-sufficiency for their health and emotional well-being while helping to remove barriers
    • Provide ongoing support for health-related things including Medicaid taxi and transportation solutions, scheduling appointments, ER and urgent care requests, new arrivals needing care for primary care visits or urgent medical care.
    • Equip clients to appropriately address medical bills and statements received
    • Facilitate access to and understanding of the resources available through the Affordable Care Act
    • Other duties as assigned

    • Medical Orientation:
    • Provide hands-on orientation to Refugee Health Promotion [RHP] clients of the U.S. healthcare system through each appointment, including proper filling and following of prescription instructions
    • Compile medical resource binder of providers and transportation options for distribution to clients and go over these resources.
    • Educate clients regarding Western medical practices and preventative care
    • Administer pre and post-tests for each adult receiving orientation Provider Relationships
    • Develop strong, collaborative relationships with health care providers and identify new sources of care in areas that lack adequate providers (such as dental care or other specialties).

    • Administrative Responsibilities:
    • Gather client documentation and create spreadsheets
    • Coordinate with other WR departments to provide strong collaborative care
    • Participate in Health Team and Family Case Management Team meetings
    • Participate in weekly case planning meetings with direct service staff and program management staff
    • Ensure monthly reporting information is submitted in a timely and accurate manner, and all RHP and R&P program requirements are met
    • Actively participate in all staff meetings, staff development opportunities, program trainings, community activities, and organizational events
    • Case note each interaction with clients daily through WR’s data management system
    • Maintain accurate records of client files, new providers, use of interpretation, and reporting records

Knowledge, Skills, & Abilities

    • Highly self-motivated with strong follow-through and ability to work independently
    • Strong mediation and problem-solving skills
    • Well organized, with an ability to manage concurrent deadlines and priorities and meet deadlines
    • Holds a learner’s posture with a strong ability to work collaboratively with others
    • Direct-service experience with vulnerable populations preferred
    • Strong interpersonal and verbal communication skills
    • Ability to work a flexible schedule including occasional early mornings and evenings, and some weekends if needed
    • Knowledgeable in State and/or local Human Services Systems and safety nets.
    • Bilingual ability desired, preference given to applicants with ability in one of the following languages: Dari, Pashto, Arabic, Burmese, and/or Russian

Requirements

    • Mature and personal Christian faith

    • Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Evangelicals For the Health of The Nation document
    • Competency in using IT systems including MS Word, Excel, Outlook, and other digital tools and the internet
    • Cross-cultural experience required
    • Must hold a bachelor’s degree in social work, public health, or related field
    • Valid driver’s license, regular access to a vehicle, and ability/willingness to drive 15-passenger van required
$19 - $23 an hour
