ORGANIZATION SUMMARY

POSITION SUMMARY:
World Relief seeks a Refugee Health Promotion Program (RHP) Specialist who will create opportunities to increase health care (including mental healthcare) literacy for eligible Afghans clients, empowering them to make informed health decisions. This position will provide medical/mental healthcare management and healthcare education to eligible Afghans clients. The RHP Specialist works closely with eligible clients to promote successful community integration and long-term self-sufficiency.
ROLE & RESPONSIBILITIES:
- Collect and review client documentation, and assess client’s eligibility for services
- Conduct client’s health needs assessment and determine medical/mental health needs and client’s current access to health care services.
- Create service plans to address all needs and overcome any barriers to health care services
- Conduct monthly group health care classes focusing on specific health related topics
- Conduct individual health related orientation to educate clients about American health care system
- Empower clients while providing them with tools necessary to meet their goals
- Provide necessary case management services to clients to assist clients to achieve basic self-sufficiency benchmarks and to meet goals of client service plans
- Build strong relationship with local clinics, hospitals and other health care resources and connect clients to these resources
- Conduct regular check-ins including home visits for active cases to assess goals’ progression, evaluate family stability and needs, and provide support and guidance to clients through case management.
- Provide transportation to initial doctor appointments, benefit interviews, and trainings for clients, as needed
- Provide referrals to community agencies and organizations for clients seeking services that WR does not offer; follow-up on referrals to ensure that client was able to connect with the recommended resource
- Maintain timely and accurate client files, case notes, and reporting records
- Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
- Work closely with internal service teams and external service providers to ensure comprehensive service provision
- Enter client information into appropriate databases
JOB REQUIREMENTS:
- Mature and personal Christian faith
- Committed to the mission, vision, and values of World Relief
- Desire to serve and empower the Church to impact vulnerable communities
- Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Evangelicals For the Health of The Nation document
- Cross-cultural experience strongly preferred
- Valid driver’s license

- Detail-oriented and organized
- Demonstrates initiative, flexibility, and responsibility
- Ability to communicate clearly, both orally and in writing
- Effectively manage stressful or crisis situations
- Fluency in Pashto/Dari is strongly preferred, but not required
- Advanced competency in the use of Microsoft Office
$22 - $22 an hour
Compensation is commensurate with experience.
