ORGANIZATION SUMMARY

This position is reliant upon funding and may be subject to modification or termination based on resource availability.
POSITION SUMMARY:
World Relief seeks a Intensive Case Management (ICM) Specialist. This position will provide intensive case management to Refugees, Ukrainians Humanitarian parolees, Cuban and Haitian Entrants in Fort Worth and Dallas areas. This role will provide support for eligible clients during their first months in the United States and make referrals for ongoing support in their communities to help them to achieve basic self-sufficiency benchmarks and to meet goals of client service plans. The ICM specialist works closely with eligible clients to promote successful community integration and long-term self-sufficiency.
ROLE & RESPONSIBILITIES:
- CLIENT SERVICES
- Conduct initial client intake to collect demographic information, gather client documentation, and assess program eligibility
- Conduct comprehensive family assessment with each client, develop individual client service plans based on strengths and needs, and facilitate connections to appropriate services and resources
- Provide necessary case management services to clients to assist clients to achieve basic self-sufficiency benchmarks and to meet goals of client service plans
- Conduct regular check-ins including home visits during the first 90 days to assess goal progression, to evaluate family stability and needs, and to provide support and guidance to clients through case management.
- Empower clients while providing them with tools necessary to meet their goals
- Provide participants and their sponsors with resources and training as necessary to ensure core services are completed.
- Provide transportation to initial doctor appointments, benefit interviews, and trainings for clients, as needed
- Provide referrals to community agencies and organizations for clients seeking services that WR does not offer; follow-up on referrals to ensure that client was able to connect with the recommended resource
- Maintain client’s service plan active for up to 12 months.
- ADMINISTRATIVE RESPONSIBILITIES
- Maintain timely and accurate client files, case notes, and reporting records
- Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
- Work closely with internal service teams and external service providers to ensure comprehensive service provision
- Enter client information into appropriate databases
JOB REQUIREMENTS:
- Mature and personal Christian faith
- Committed to the mission, vision, and values of World Relief
- Desire to serve and empower the Church to impact vulnerable communities
- Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Evangelicals For the Health of The Nation document
- Cross-cultural experience strongly preferred
- Have a valid Driver's License

- Excellent interpersonal and communication skills
- Ability to manage multiple demands and deadlines with flexibility
- Ability to maintain records and reports
- Detail-oriented and organized
- Demonstrates initiative, flexibility, and responsibility
- Ability to communicate clearly, both orally and in writing
- Effectively manage stressful or crisis situations
- Fluent in Spanish is strongly preferred, but not required
- Ability to speak a second language is preferred, but is not required (Spanish or Haitian Creole preferred)
- Advanced competency in the use of Microsoft Office
$22 - $22 an hour
Compensation is commensurate with experience.
