Title: Care Coordinator

Overview:

Cleo is looking for a highly organized, proactive, and service-oriented individual to join our Government Health Plans team as a Care Coordinator on a part-time, hourly basis (up to 25 hours per week, with shifts scheduled between 8am–8pm EST; specific schedules, including potential rotating shifts, will be determined during hiring/onboarding). In this role, you’ll play a critical part in supporting the day-to-day operations of Cleo’s care model. You'll ensure seamless coordination across member touchpoints by managing scheduling logistics, monitoring high-priority follow-ups, and supporting internal teams with timely information and ad hoc requests.

You’ll work closely with Coaches and Engagement Specialists to ensure every member interaction is thoughtful, timely, and aligned with their unique needs. The ideal candidate is someone who thrives in a fast-paced environment, excels at clear communication, and brings a solutions-oriented mindset to every task.

This role is perfect for someone who is passionate about operational excellence, customer support, and contributing to a mission-driven team focused on transforming the experience of aging.

Key Responsibilities:

    • Source accurate, high-quality resource and care options across a wide range of member needs. Focus areas include adult and eldercare, healthcare and benefits navigation, emotional wellness, support with ADLs/IADLs, end-of-life planning, health related social need-risk resolution, household tasks, and administrative, financial, and legal services.
    • Manage scheduling requests and changes on the Government Health Plans care team to ensure sessions are coordinated accurately and promptly.
    • Manage inbound calls received through Cleo’s Health Plan phone line and promptly route them to the appropriate team members for efficient handling.
    • Monitor inbound Health Plan referrals through the team’s shared email inbox, missed member calls, and voicemails, and communicate high-priority follow up needs with appropriate internal team members to ensure prompt outreach.
    • Access and manage documentation in internal or customer systems to ensure accurate and timely updates to member profiles.
    • Provide high quality customer service to Cleo members.
    • Support the Government Health Plans Member Operations team with other ad hoc needs as they arise.

Qualifications:

    • 1-3 years of experience in an administrative or customer service role, ideally with exposure to family-centered programs. Experience in social services, care coordination, or healthcare navigation is a plus.
    • Strong research skills, existing knowledge, and sound judgment in evaluating community and Health Plan resources for quality, accessibility, cultural fit, and insurance compatibility. 
    • Experience working with diverse populations and navigating complex care systems.
    • Exceptional verbal and written communication skills, with the ability to tailor your communication style to different audiences.
    • Attention to detail, especially regarding logistics, customer service, and coordination.
    • Strong organizational skills, with the ability to be proactive and collaborate as part of a team.
    • A passion for building member experiences and maintaining a positive, team-driven approach, even under pressure.
    • Comfort with professional tools such as Google Sheets, Google Drive, ticket management systems (e.g., Front App), Slack, and Google Calendar in a professional setting.
    • Background in or passion for working in health services related to government healthcare programs or insurance.

Pay Transparency: The base rate for this position is $20-25 per hour for up to 25 hours per week. Additional benefits include 15 paid holidays, 5-day winter break, accrued vacation time, sick time, 16 weeks paid parental leave, 401(k) plan and more.
